Please note that should you be successful, the Under 18 State Team Trials for Men and Women are scheduled to be held on the following dates:

• Country – 1-2 October, WABC
• Metro – 9 October, WABC

This application form is to be filled out by all applicants and must be filled in for Basketball WA to accept your application.
